Start A JSA
One JSA per crew, per task, per day. Re-brief and update whenever scope, personnel, equipment or conditions change.
Submit Paper JSA
For a JSA you completed on paper. Photograph every page, including the signatures. It goes to your supervisor for review and approval exactly like one filled in here.
Optional on purpose. A crew with no signal does not owe an explanation, and a required box here would be one more thing between them and a filed JSA.
Stopped — Re-Plan Required
Work does not start. Re-plan the step so its residual risk is no longer High, or contact the Field Service Manager.
Sent Back For Revision
A reviewer has asked for a correction before this JSA can be approved. The work is finished and the record of it stands — only the document is being corrected.
Make the correction in the sections below, then send it back. §1, §3, §5 and §6 are open again for this. §2 and §4 change through a re-brief, as always.
Required. The reviewer sees this instead of comparing two versions by eye.
This returns the JSA to Pending Review and tells the reviewer who sent it back. It does not change when the day started or finished.

Emergency Information
Section 3 asks whether the emergency plan was reviewed. This is where what you reviewed gets written down, so the crew can read it later with no signal. None of it is required, and a blank is better than a guess.
Conditions At Start
Record all three. Enter 0 rather than leaving a box blank.
Tick every hazard that applies. If it is clear skies with no weather issues, tick No Weather Hazards instead — that records that you checked, which a blank section does not.
Work-At-Height / Lift Wind Limit
Work stops when the limit is reached. PULSE checks the wind and gust readings above against it.
If your manual or customer requirement states a decimal, round down, because a lower limit stops work earlier.
2. Crew Participation And Competency*
Every person on the task signs before work starts. Each person signs in turn on this device. A contractor with no PULSE account is added the same way.
3. Pre-Job Checks*
Complete on site, this shift, before work starts. Answering N requires a note explaining what was done instead.

4. Task Steps, Hazards And Controls*
Break the job into steps in the order they happen. State what will physically be done, not "be careful". If any step is still High after controls, work does not start.
5. Critical Control Verification
These are the controls that stop a fatality. The groups shown are required by the mechanism codes on your task steps.

Complete And Brief
Crew Lead: sign to confirm the JSA is complete and briefed. Work may begin once this is signed.
Briefed
§1 conditions at start and the §3 / §5 / §6 verifications are what the crew signed and stay as briefed. A task step or a crew change records a §7 re-brief above, which the whole crew acknowledges. Close the day out in §8 when the work is finished.
