Observations
Say whether you saw safe work or a chance to learn, describe it, record what was safe and what was at risk across eight hazard categories, and say what happened next. This is not a disciplinary form and it never records the name of the person observed. You can report an observation anonymously, and then no name is stored. It is used to fix hazards and spot trends.
